Mosca: SSJ100 Aeroflot rientra col fuoco in coda

Russian investigators are analysing flight-recorder data to understand what happened in the critical 20min interval between a Sukhoi Superjet 100 apparently being struck by lightning and its being destroyed during a hard landing at Moscow Sheremetyevo airport.

The aircraft (RA-89098) experienced a "discharge of atmospheric electricity" at 6,900ft as it climbed out of Sheremetyevo for Murmansk on 5 May, says federal air transport regulator Rosaviatsia.

Meteorological data at the time indicated cumulonimbus cloud, associated with thunderstorm activity, at 6,000ft.

The aircraft's radio and other equipment failed and the captain opted to return to the airport, setting the squawk code to 7600 to signify loss of communication before transmitting a distress signal.

However, the full details of the aircraft's controllability over the subsequent 20min are yet to be clarified.

It appears to have twice attempted to line up with the approach path before commencing its descent. The inquiry has yet to indicate whether navigation or instrument landing systems were functioning on the aircraft.

Video images indicate that the aircraft bounced as it landed hard on runway 24L, about 30min after departure, with the main landing-gear apparently collapsing. Rosaviatsia says a fire broke out upon the fourth contact with the runway.

Passenger video images captured upper wing surfaces burning as the aircraft slid along the runway, leaving a fiery trail, with a severity that suggests the impact damaged the aircraft's engines, fuel lines and possibly wing tanks.

The twinjet subsequently veered off the runway, yawing some 90° to the left and coming to rest at the A2 taxiway intersection about 2,700m (8,860ft) along the runway.

Analysis will examine whether evacuation was impeded by passengers' stopping to retrieve luggage

Weather data shows a southerly crosswind of 13kt (24km/h) at the time, and video shows smoke and flames from the starboard side being blown across the aft fuselage and empennage.

Survivability analysis will inevitably examine whether evacuation was impeded by passengers' stopping to retrieve cabin luggage. Video images clearly show people walking away from the burning aircraft with large cases and other baggage.

After the jet came to a stop, some 15s passed before the forward right-hand exit slide deployed followed by the left-hand slide 10s later.

Aft exits would have been unusable, owing to the intense fire surrounding the rear fuselage, and the Superjet 100 is not fitted with overwing exits - although the fire would probably have rendered them unavailable in any case.

As a result, passengers seated in the fire-hit rear section of the cabin, comprising the last 10 or so seat rows, would probably have had to wait to progress along the upward-sloping aisle while passengers in the forward rows cleared the aircraft.

Aeroflot's Superjets are typically configured with 87 seats including 12 in a four-abreast business cabin and 75 economy seats in a five-abreast layout. The aircraft operating the ill-fated flight SU1492 was carrying 73 passengers and five crew, which meant more than four out of every five seats was occupied.

Forty-one of the 78 occupants did not survive the accident.

Aeroflot claims that the evacuation was conducted within 55s, although video images show individuals - whether they are passengers or crew is unclear - descending the forward right-hand slide around 70s after its deployment, with firefighters applying extinguishing agent shortly afterwards.

In a paper published last year, the UK's Royal Aeronautical Society states that proliferation of mobile phones has shown up the problem of passengers' collecting luggage during evacuation.

SAFETY BRIEFINGS

"Some operators, in an attempt to capture passenger attention during pre-flight safety briefings, have produced clever and often quite humorous briefing routines or videos," it states. "There is some question as to whether passengers adequately recognise the gravity of such briefings, or actually understand the important safety information being provided."

Australia's Civil Aviation Safety Authority says passengers will endeavour to take belongings, especially high-value items, "particularly where danger to life is not evident".

Russian airframer Irkut has promoted the large overhead luggage bins on its new MC-21 - production of which is being brought closer to that of the Superjet 100 - but it is also offering an optional remote-controlled electromechanical lock for the bins as a safety feature.

Russian investigators have disclosed that the Sukhoi Superjet 100 involved in a fatal accident at Moscow Sheremetyevo was 1.6t over its maximum landing weight, and experienced two impacts in excess of 5g as the aircraft bounced on landing.

The Interstate Aviation Committee says it has completed an initial analysis of information from the flight-data recorder retrieved from the Aeroflot jet after the 5 May event.

Investigators have revealed that the crew received windshear warnings on approach and that the aircraft experienced pitch fluctuations just before the fatal touchdown.

Federal air transport regulator Rosaviatsia, in a detailed outline of the flight, states that the aircraft suffered an electrical failure at 8,900ft - about 5min after take-off from runway 24C - while following the KN 24E standard departure pattern for a service to Murmansk.

CONTROL SYSTEMS

The autopilot disengaged and the aircraft's flight-control system dropped into direct law.

Rosaviatsia does not specifically state that the aircraft was struck by lightning, but the regulator does point out that the aircraft was flying within a "zone of thunderstorm activity".

The flight recorder registered disengagement of the autothrottle, and Rosaviatsia says the captain then manually controlled the aircraft for the remainder of the flight.

Unable to communicate on the approach frequency, the crew restored VHF radio links using the emergency frequency 121.5MHz, and were vectored back to Sheremetyevo while transmitting the squawk code 7600 for loss of communication.

The aircraft conducted an ILS approach, in manual mode, to runway 24L.

Rosaviatsia says the aircraft had departed with a take-off weight of just over 43.5t and its weight upon entry to the glideslope was 42.6t - which, it says, exceeded the maximum landing weight by 1.6t.

As required for the overweight landing, and the direct-law control, the flaps were set to 25°. The crew also upgraded the squawk code to the emergency setting 7700.

Fatalities comprised 40 of 73 passengers and one of the five crew, while six passengers and three crew were injured Rosaviatsia

The aircraft remained largely stable on the approach - performed in a crosswind from the left of up to 30kt (55km/h) - with an airspeed of 155-160kt.

As the Superjet descended through 1,100-900ft above the ground, the crew received five predictive windshear "go around" warnings, Rosaviatsia says.

The aircraft began to dip below the glideslope at about 260ft and, at 180ft, a glideslope alert sounded.

Thrust was subsequently increased, with the throttle levers alternately advanced and retarded between 18° and 24° as the aircraft descended to 40ft. This resulted in the airspeed increasing to 164kt as it crossed the threshold and 170kt at 16ft from touchdown.

As the captain retarded the throttle to idle, says Rosaviatsia, he made several alternating inputs to the side-stick with "large amplitudes" - up to the maximum - which resulted in the pitch varying between 6° nose-up and 2° nose-down.

While the aircraft had appeared close to touchdown at about 700m (2,300ft) from the threshold, Rosaviatsia says the first three-point contact with the runway occurred at 900m from the threshold at 158kt, when the aircraft experienced an impact of more than 2.5g, and bounced to about 6ft.

Rosaviatsia says the aircraft's spoilers did not deploy automatically. Aeroflot stresses that its procedures do not require the manual deployment of spoilers until thrust-reverse is activated and the aircraft is settled and stable on the runway.

"In the absence of a stable course the release of the spoilers was impossible," the carrier adds.

DAMAGE SUSTAINED

Having bounced, the aircraft touched down 2s later on its nose-gear at 155kt, with a heavy impact of 5.85g, causing the Superjet to bounce a second time, to a height of 18ft. The third, and final, impact occurred at 140kt - reaching at least 5g - and was immediately followed by damage to the aircraft's structure, a fuel spill and fire.

As the aircraft decelerated through 100kt, sliding along the runway, a fire alarm was triggered in the aft baggage and cargo compartment, followed by a fire alarm in the auxiliary power unit 16s later. The aircraft's PowerJet SaM146 engines continued operating until the end of the flight-data recorder trace just after 18:31.

Rosaviatsia says the captain had logged 1,570h on type out of a total of 6,844h, while the first officer had 623h on type.

The aircraft (RA-89098) had accumulated 2,710h over the course of 1,658 cycles.

Rosaviatsia says the fatalities comprised 40 of the 73 passengers and one of the five crew members, while six passengers and three crew were injured.

Aeroflot stresses that the preliminary information disclosed by Rosaviatsia does not reference errors by the crew or any violation of procedures, and that final conclusions have yet to be released by the investigating authorities.

Criminal investigators have charged the captain of an Aeroflot Sukhoi Superjet 100 in connection with the fatal landing accident at Moscow Sheremetyevo airport earlier this year.

The Murmansk-bound aircraft returned to Sheremetyevo after a lightning strike but bounced heavily during the landing, igniting spilled fuel when the landing-gear collapsed.

Forty-one of the 78 occupants died in the accident.

The captain had taken manual control of the Superjet after the lightning strike, and the investigation has highlighted heavy-handed and abrupt inputs to the side-stick during the approach and touchdown.

Russia’s Federal Investigative Committee says that the aircraft made a rough landing, adding that the captain’s actions to control the jet were “in violation of established rules” and led to the airframe damage and the outbreak of fire.

It states he is being charged with a crime relating to breaches of air transport safety rules which, as a result of negligence, resulted in two or more fatalities.

The committee says 10 people on board received injuries of varying severity, adding that the criminal investigation is continuing.
